The following checklist should help you better plan for your initial visit to our clinic:

  • Intake forms will need to be completed. The new patient information form and HIPAA form are aavailable on our website for download and completion. Active Spine Physical Therapy’s privacy policies for HIPAA compliance are also available on our website for your review.
  • Bring your insurance cards to be copied for your medical record in order to ensure accuracy when filing your insurance claims.
  • Bring your photo id or drivers license so that we can copy it for your records to help protect your identity.
